Signature Release Form:
After you submit your application to TSMT, certain states require your signature on a release form permitting us to research your previous driving experience. This signed release may be required in order for us to proceed with your application, your recruiter will let you know. The form can be downloaded from the link below, printed, signed, and faxed to 888-888-0962. If you would rather us fax or mail you a copy of the form, please call the recruiting department at 800-846-8768. Thank you.
